Privacy Policy


This policy is designed to ensure that information gathered by your counsellor is used in an appropriate, secure manner and to provide access to and correction of such information.


Information Gathered
IASO CARE gathers information that allows IASO ACRE to make informed professional decisions regarding the most appropriate care to offer. This information will include basic demographic data (e.g., name, date of birth, address, etc.) as well as other information that is deemed relevant. This may include the following:


  • The nature of the problem for which you are presenting;
  • Areas of behaviour related to the presenting problems;
  • A history of the presenting problem;
  • A family history of relevant information; and
  • Other information deemed necessary to make an informed clinical judgement about the nature of the presenting complaints and other relevant factors


Confidentiality of Information
Generally, all information gathered by IASO CARE within the service remains confidential. There are several notable exceptions, however, to this confidentiality:


  • In the event that the counsellor forms the professional opinion that either the client or another individual is at risk for significant harm, a moral obligation exists to act on this information with the aim of preventing such harm;
  • In the event that a file is subpoenaed to a court,
  • If a written request is received from the client, including the client’s signature.


Use of Information
Information gathered by the IASO CARE is used exclusively for two purposes:

  1. For the formation of a professional opinion, which is used to guide treatment for the presenting issue, and,
  2. To evaluate the service provided by IASO CARE through the collation of detailed statistics about referral sources, nature of presenting problems, attendance durations, etc.,

No identifying information is made available in all cases where information is used for statistical purposes.


Security of Information

Information provided to the IASO CARE is held in the client file and IASO CARE's database (demographics and basic diagnostic information only). All files will be stored securely and can only be accessed by team members of IASO CARE. 


Obtaining Recorded Client Information
Client information will only be released in accordance with the "Confidentially of Information" section above.
Requests for information may be made to the IASO CARE. Requests should be specific to the required information and will be acknowledged within 30 days. Once a request for information is received, IASO CARE will assess whether the request meets the criteria outlined above and conduct a thorough chart review, collating the requested information appropriately to ensure adequate and full understanding on the part of the requesting person.


Procedure for Complaint/Rectifying Information
Official complaints or requests to change inaccurate or erroneous information should be made to IASO CARE. In such cases, IASO CARE will review the information held by IAOS CARE and make every effort to ensure that such information is accurate.


Access to the privacy policy
This document is freely available and accessible on request by all clients of IASO CARE on the IASO CARE's website.
